Everything you need, in one place.

Forms to read or sign, the client portal, a questionnaire about childhood experiences, and where to turn in a crisis.

Forms & Portal

Every consent form to read or download, in English and Spanish, and the portal for signing them online, seeing statements and booking appointments.

Crisis Support

If you are in crisis, call or text 988 any time, day or night. For Spanish, call 988 and press 2, or text AYUDA to 988. In an emergency, call 911.

Learning Resources

Short introductions to what therapy and recovery involve.

  • Therapy: a place to understand patterns, work through hard experiences and build healthier responses, at your own pace.
  • Adverse childhood experiences (ACEs): hard events in childhood, such as abuse, neglect or trouble at home. An ACE score is not a diagnosis.
  • Trauma and recovery: trauma can affect sleep, mood, focus and relationships. Recovery is personal.
  • Parenting: safety, steady routines, listening and repair after conflict help children grow.
  • Relationships: listening, saying what you need, respect, boundaries and room for repair.

Community Resources

Beyond our own programs, these Arizona organizations help with food, health coverage and legal needs. They're run by other organizations, not Confianza Health.

More Crisis & Support Lines
